Pet Information Form for Pet Sitters

Pet sitters should require each client fill out this Pet Information Form, one for each pet that they are looking after. The client must provide information about:

• the pet's physical description, character, personality, likes and dislikes, favorite toys, sleeping spots, etc.,
• the food, feeding schedule, and special dietary needs of the pet,
• house training issues, toilet habits, behavior issues,
• the pet's medical history, health problems, medication, vet information,
• locations in the client's home where food, medicines, cleaning supplies, leashes, etc. are kept,
• social behavior of the animal, events that spark fear or anxiety, and any other special conditions.

Buy and download the Pet Information Form and hand it out to each new client that you sign up for your pet sitting services.
