This form is a revocation of the power and authority granted in Form WI-P010 that grants your attorney in fact (your agent) VERY broad powers with regard to your property and finances. This form must be signed and notarized. You may revoke a power of attorney for finances and property and invalidate it at any time by destroying it, by directing another person to destroy it in your presence or by signing a written and dated statement, such as this form, expressing your intent to revoke.
