Special Power of Attorney (Utah)

This is a statutory form that is provided in the Utah Code. It allows you to appoint an attorney in fact (an agent) to execute a directive on your behalf under Section 75-2-1105, governing the care and treatment to be administered to or withheld from you at any time after you incur an injury, disease, or illness which renders you unable to give current directions to attending physicians and other providers of medical services.
