Revised Uniform Anatomical Gift Act Donation (Utah)

This Revised Uniform Anatomical Gift Act Donation form pursuant to state statutes designates the specific body parts and organs an individual wishes to donate at the time of death. This form should be witnessed and the signature notarized. An individual who is 18 years of age or older may make an anatomical gift of all or any part of his own body to be used for transplantation, therapy, medical or dental education, research, or the advancement of medical or dental science.
