Durable Power of Attorney for Health Care with Advance Health Care Provisions (Pennsylvania)

This Pennsylvania Durable Power of Attorney designates a health care agent to express health care desires on your behalf when you are not capable of doing so yourself. You may specify any limitations on the types of decisions the health care agent may make on your behalf. It also includes living will or advance health care declaration provisions with regard to specific life sustaining procedures and treatment. This form is used to direct the attending physician to withhold or withdraw life-sustaining treatment that serves only to prolong the process of dying if you are in a terminal condition or in a state of permanent unconsciousness.
