Petition to Reinstate Driving Privilege

This is a Petition to Reinstate Driving Privileges. This form is used when the Defendant did not submit to a field sobriety test, and had his/ her license revoked. Consequently, he/ she was found not guilty of driving while intoxicated, but was still refused his/ her license by the Department of Public Safety. This Petition asks the Court to review the Department of Public Safety's decision in this matter. This form is applicable in all states. (more...)
