Essential Documents for the Organized Traveler Package (Georgia)

This package contains forms essential for persons who travel. It is designed to assist in travel preparations, help minimize difficulties that may arise while traveling, and keep important information organized. Don't leave unprepared.

Included in your package are the following forms:

Traveler Information Form - This form allows the traveler to keep their important travel information on hand for easy reference, such as passport, itinerary, airline, hotel, car rental, and other information.

Statutory General Power with Durable Provisions - Financial - This is a statutory form provided for in the Georgia Code that allows you to name one or more persons to help you handle your financial affairs. It grants your attorney in fact (your agent) VERY broad powers with regard to your property and finances. This Power of Attorney does NOT provide for health care services. As long as you are living, the financial power of attorney will remain in effect even if you become incapacitated or unable to communicate your wishes unless a guardian is appointed for your property or you include a date or specific occurrence when you want your document to be canceled.

Passport Application - This form is used to apply for a new passport, as opposed to renewal of an existing passport.

Passport Renewal Application - This form is used to renew an existing passport.

Pet Boarding Agreement - This form is an agreement between a pet owner and a pet boarder.

Travel Expense/Reimbursement Form - This form is used to request a reimbursement of funds used by an employee for business travel.

Statutory Advance Directive for Healthcare - This Statutory Advance Directive for Healthcare allows you to choose someone to make health care decisions for you when you cannot (or do not want to) make health care decisions for yourself and for you after your death with respect to an autopsy, organ donation, body donation, and final disposition of your body. You can also use this form to state your treatment preferences if you have a terminal condition or if you are in a state of permanent unconsciousness, and to nominate a person to be your guardian should one ever be needed.

Babysitter Emergency Information Form - This form is an important tool to ensure the safety of your children while in the care of a babysitter. Vital contact, medical, and emergency information can be provided to easily reference if needed. (more...)
