Statutory Uniform Anatomical Gift Act Donation by a Living Minor (Delaware)
This is an Anatomical Gift Form which designates the body parts and organs that a minor (under 18 years of age) wishes to donate at the time of death. This form provides for the necessary parental consent to effectuate such a gift. See Delaware Code 16-2710 to 16-2719.
