Document Concerning Withholding or Withdrawal of Life Support Systems (Connecticut)

This is a statutory form that is provided for in the General Statutes of Connecticut that allows you to express your wishes and desires regarding whether or not your life is prolonged by artificial means. Any person eighteen years of age or older may execute a document which contains directions as to specific life support systems which that person chooses to have administered.
