There has been much debate over access to employment benefits for domestic partners. A number of laws have been enacted at the state level that allow domestic partners to have the same rights as a spouse regarding inheritance, probate, guardianship and other rights traditionally accorded to spouses. Insurance matters would also arguably be included. Today, a large number of private employers, nonprofit organizations, and unions in the United States offer domestic partner benefits to homosexual or unmarried heterosexual partners.
This form is a generic example that may be referred to when preparing such a form for your particular state. It is for illustrative purposes only. Local laws should be consulted to determine any specific requirements for such a form in a particular jurisdiction.
