Each time you draw a progress payment on a construction project, use this form to prepare an Unconditional Waiver and Release of Lien for the owner or project manager.

• This form is for the State of Washington.
• Don't use this waiver and release unless the check for the payment has actually cleared. If not, use a Conditional Waiver and Release instead.
• The contractor makes representations that all subtrades, suppliers and employees have been paid up to the date of the payment.
