• Don't use this form unless and until the final check has been endorsed and has cleared the bank.
• The contractor attests that all invoices for labor, materials and services on the project have been paid, or will be paid from the proceeds of the final check.
• You can re-use this form as often as you need to. Fill it in, print it, sign it.
