Utah Notice to Tenant of Tenancy Termination for Non-renewal of Tenancy

Utah landlords, if you and your tenant are not renewing the rental agreement for the tenant's rental unit, you must serve the tenant with this Notice to Tenant of Tenancy Termination for Non-renewal of Tenancy.

The Notice advises the tenant that the rental agreement will not be renewed, and requires the tenant to vacate the premises by a specified date or the landlord has the right to commence eviction procedures.
This form of termination notice is for fixed term tenancies (such as 6 months, 1 year, etc), and NOT for monthly or weekly tenancies.
