If you've been appointed as attorney under an LPA and you don't wish to act, you can register this Disclaimer by Proposed Attorney or Acting Attorney under a Lasting Power of Attorney with the Office of the Public Guardian.

• This is Form LPA005, issued 2011 by the Public Guardian.
• The form can be used to stop an appointment under a new power of attorney, or to discontinue acting under an existing power of attorney.
• Valid for use in England and Wales.
