Ohio Notice of Commencement

Ohio property owners, you must give notice of improvements on your property to all interested parties (such as a mortgage lender) by serving them with this Notice of Commencement form.

• The Notice of Commencement is an affidavit sworn by the owner, part owner, or lessee of the property.
• It must be sworn and served in accordance with Ohio Revised Code Section 1311.04.
• All parties with an interest in the property are notified of the first day that improvements will begin.
• The notice also gives relevant information about the contractor, the surety, the property owner, and the work to be performed.

You can download the Notice of Commencement form immediately after you purchase it. Then fill it in and sign it in front of a notary.
