Kentucky Notice to Tenant of Termination of Rental Agreement and Notice to Vacate

Kentucky landlords, if you have reason to terminate a residential tenant, serve them with this Notice to Tenant of Termination of Rental Agreement and Notice to Vacate, in accordance with the Uniform Residential Landlord and Tenant Act.

The notice period is to be for the number of days as stated in the rental agreement (eg. 30, 60 days, etc.).
The tenant is notified that the landlord will terminate the rental agreement on the date specified in the Notice, and must vacate the premises by that date.
A Service of Notice form is included, to record the details of how the Notice was served on the tenant.
