- The Notice informs the landlord that the tenant intends to vacate the premises because the landlord failed to deliver the premises in compliance with applicable housing and building codes that materially affect the health and safety of tenants.
- The tenant also requests reimbursement for any additional rent that he/she must pay at a new residence in excess of the rent the tenant was paying for the landlord's rental premises.
- The tenant requests the return of the security deposit within 30 days.
