Anyone who serves a notice on a person or firm in California must complete a Proof of Service of Notice afterwards.

• The person who served the notice must record the date of service and the method used to serve the person(s) - whether by personal service or by mail.
• The Proof of Service is evidence that the notice was served on the receiving party.
