Notice of Assignment of Debt to Debtor

This Notice of Assignment of Debt to Debtor will notify a debtor that the entity who currently owns the debt (such as a mortgage company or bank) has assigned debtor's account to another party. This notice sets out the name of the debtor, name of the entity to whom the debt was assigned and the amount owed. It also provides the address and phone number of the assignee.
