Guardianship Petition

This Nevada Temporary Guardianship Agreement will effectively award guardianship of a minor child without going through a court proceeding. This agreement contains the name of the legal guardian of the child, the individual who will be awarded guardianship on a temporary basis and the name and birth date of the minor child. It also sets out the matters over which the guardian has control, including health care decisions, education and general welfare. This agreement takes effect immediately and will expire six months from the date of signing unless otherwise renewed. (continued ...)
