This Letter to Other Entities Notifying Them of Identity Theft is used by an identity theft victim to notify various entities of identity theft, including a school or loan program for fraudulent student loans; a phone service provider for fraudulent use of telephone, cell phone and/or long distance accounts; the U.S. Department of State, Passport Services for fraudulent use of passports; and the Federal Trade Commission Identity Theft Clearinghouse for inclusion in the database of identity theft, which includes information that is shared with law enforcement agencies for investigation.
