Waiver and Assumption of Risks

This Waiver and Assumption of Risk is for use in situations where a customer or event attendee is taking a risk when using a business' facilities (such as a gym), receiving instruction or participating in an event (such as a sports tournament or bicycle marathon). This waiver contains the name of the individual and owner of the business or event sponsor. It also contains language setting forth that the individual understands the risks and dangers associated with the activity or use of the facilities and agrees to assume the risks for all injuries or damage.
