Minor Release Agreement

This Minor Release is written consent by the parent or legal guardian of a minor child given to another which grants permission to take photographs or record the voice of the child in connection with a motion picture. This Release sets forth the express terms of the agreement and further warrants that the child was a minor at the time of signing.
