Affidavit of Execution

This Affidavit of Execution sets forth that the individual executing this affidavit personally witnessed the signing of another document. This affidavit sets forth the names of the individuals who signed the document, that they were over the age of eighteen when the document was signed and that affiant was the subscribing witness to the original document.
